MINOR MEMBERSHIP AGREEMENT
In accordance with the Federal Trade Commission's rules on Children's Online Privacy Protection, the Photo Martini Club, SHOT! Magazine, Gallery Photo Martini Club and Studio Photo Martini Club have provided the following information.

OPERATOR COLLECTION INFORMATION
All information collected and stored by the Photo Martini Club, through web site use or participation in club activities, is maintained by:

INFORMATION COLLECTED
The Photo Martini Club collects personal information including:

• name
• address
• phone
• email
• birthdate

The Photo Martini Club will not require a child to disclose more information than is reasonably necessary to participate in an activity as a condition of participation.

USE OF INFORMATION COLLECTED
Personal information is used solely for the operation of the Photo Martini Club activities.

Addresses are used for the mailing of SHOT! Magazine, membership materials, and prize distribution.

Email and phone numbers are only used for club communications, unless the member has requested direct communication by potential buyers of photography.

In the case that the photographer is a minor, web site profiles and magazine pages will NOT display any personal information other than the name of the photographer and all phone and email communications are copied to the parent or legal guardian.

DISCLOSURE OF PERSONAL INFORMATION
The Photo Martini Club does NOT sell or trade member information for marketing or any other purposes. Disclosure of personal information to third parties is only granted at the request of law enforcement or federal regulatory agencies, and only then with the proper documentation and through proper legal channels.

REVIEW OF INFORMATION
At any time the partent or legal guardian of the minor registered with the Photo Martini Club may request:

• a full review of the information on file
• that any or all information be deleted from the Photo Martini Club files (understanding that it may result in reduced benefits or participation in club activities)

CHANGES TO POLICIES
The Photo Martini Club agrees to inform parents and legal guardians of any changes to these policies at least 30 days prior to their implementation. If such changes to policy are not acceptable to the parent or legal guardian, then any and all information collected will be removed form all Photo Martini Club files and the membership shall be dissolved.
